Is menopause hormone therapy (MHT) a first-line therapy for the prevention of bone loss?

In International Menopause Society (IMS) Recommendations and Key Messages on Women’s Midlife Health and Menopause: 6. Osteoporosis, published 23 December 2026, the authors include:

  • “MHT is first-line therapy for the prevention of menopause-related bone loss. Benefits are most likely to outweigh any risks when initiated within 10 years after menopause or before age 60 years. Cessation of therapy leads to rapid bone loss. MHT significantly reduces the risk of osteoporosis-related fractures. ⨁⨁⨁◯ A”.2

Bisphosphonates

What are bisphosphonates?

DotS the definition of bisphosphonates may vary. In International Menopause Society (IMS) Recommendations and Key Messages on Women’s Midlife Health and Menopause: 6. Osteoporosis, the authors’ definition is:

  • “Bisphosphonates (BPs) are potent inhibitors of bone resorption with proven efficacy in the prevention of all osteoporosis-related fractures. It is indicated in women at high and very high risk of fracture… ⨁⨁⨁⨁ A”.3

In International Menopause Society (IMS) Recommendations and Key Messages on Women’s Midlife Health and Menopause: 6. Osteoporosis the authors also note:

  • “All postmenopausal women should be educated on a bone-friendly lifestyle, including physical activity (resistance/weight-bearing exercise and falls prevention) and diet containing the recommended calcium intake and vitamin D. This is also an essential adjuvant to any pharmacological intervention. ⨁⨁⨁⨁ A”.4
